The types of questions available in Microsoft forms are Choice, Text, Rating, Date, Ranking, Likert, File upload, and Net Promoter Score as shown below. Based on these question type, I do not think forms can break out the address the way you mentioned, it would need separate questions to collect Street, Apartment etc.

Turn on Save and fill addresses. The setting is on when it has a blue background, and it's off when the background is white.

We have a requirement to include a third party reporting tool add-in ( Vision for Excel ) within MS Excel 2010. As the screen below shows, the INFOR Q&A add in in included and is shown as under active application in the add-in menu, it do not get shown in the excel task bar.
